That's the most complex question in business VoIP.
It's all about
QoS.
What models of switches and routers are you using? What QoS features do they support? What QoS features is your VoIP vendor recommending?

Outside calls will be routed over your broadband Internet connection to your VoIP service provider.
Of course, that means that will be reevaluating the size of your Internet connection!
Alternatively, you
could just use VoIP internally and then connect to the POTS are your PBX, but I'm not sure that I see the business value in that.
